Preparing for your first appointment
- Arrive 15 minutes early for your first appointment to allow for time to fill out paperwork.
- Got questions? Make a list of questions you want to discuss on your first visit.
- Bring all information with you. This includes a copy of your insurance card and a photo ID.
- Please call our office ahead of time at 918-303-5633 to verify your insurance benefits.
- Be prepared to share your medical history, previous treatments, current medications, and other pertinent information.
- Make a list of any signs or symptoms that the practitioner should know. What has been irritating or hurting you lately? How long have you been dealing with the issue? What does it prevent you from doing?
Fill your paperwork out beforehand to save time!
If you wish to save time on your first visit, please print the patient forms below, fill them out, and bring them with you to your appointment. If you don’t have time to print them ahead of time, arrive at least 25 minutes early to give yourself plenty of time to finish them before your appointment.

The APTA’s website states, “Direct access means the removal of the physician referral mandated by state law to access physical therapist services for evaluation and treatment. Every state, the District of Columbia, and the US Virgin Islands allow for evaluation and some form of treatment without a physician referral.”
In other words, this means you can choose what clinic you want to receive treatment from without having to wait for your primary care doctor to refer you to them. No referral or prescription is necessary for this!
